Declaration

In order to be considered as a foster carer for Camden Council the following requirements must ber met. (tick the box if you meet the requirment)

Live within the Camden Local Government Area (LGA)

Be over 18 years of age

Be able to adhere to all of Camden Councils guidelines, policies and instructions regarding foster care

Be able to provide daily care for cats and/or kittens

Have a suitable indoor space to house the cat or kittens

If other people are in the house, they must not be allergic to cats

If other people are in the house, they must have no history of animal cruelty

I declare that I have never:

Been accused/convicted/fined for the mistreatment of any animal

Been suspended or prohibited from purchasing, acquiring or having animals under my care, control or supervision

Had an animal under my care be seized

I acknowledge that:

an inspection is required to be completed by Council Officers in order to determine initial suitability as a foster carer

additional inspections are required to be completed by Council Offers in order to determine on going suitability as a foster carer order continue as a foster carer

I have read and understand the requirments as set out in the Foster Care Manual and Agreement
